If you are the carer of someone living with a dementia who is coming into hospital, we will ask you to fill in a ‘Knowing Me’ form to tell us more about them.
Knowing Me

The form is an easy and practical way of recording who they are as a person. It lets health and social care professionals see them as an individual and deliver person-centred care that is tailored specifically to their needs.
The form includes space for details about the person’s cultural and family background; events, people and places from their lives; preferences, routines and their personality.
It is suitable for use in any setting – at home, in hospital, in respite care or a care home – and can help reduce distress for the person with dementia and for their carer.
It can also help to overcome communication difficulties and prevent more serious conditions developing, such as malnutrition and dehydration.
