Learning Disability Liaison Nurses
The hospital learning disability liaison nurses help to meet the care needs of patients with learning disabilities
The team help people by:
- Promoting health education
- Help people to understand their treatment
- Preparing for hospital visits or stays
- Help make sure things go well in hospital
- Helping hospital staff to communicate well
All patients with learning disabilities should have a My Care Passport which is up to date. If you need help or advice with how to complete this document please contact the Learning Disabilities Nurse Team (numbers below)
