Congratulations on your Conditional Offer of Employment with Western Sussex Hospitals NHS Foundation Trust
We want to ensure that everything is in place prior to you starting your new position with the Trust. In order for us to ensure this, we require you to attend an Identification Appointment with us before your proposed start date.
You can book the next available appointment now by following the link in your Conditional Offer Letter.
The Offer Letter will also provide instructions and information regarding the documentation you are required to bring with you to your appointment.
We need all completed paperwork / documentation at least six weeks prior to your start date so that we have time to process your details; therefore you may need to complete and return the below listed paperwork before your ID appointment. If so, please return by email (scanned) or posted for the attention of the named person stated in your offer letter.

- P46 Starter Checklist
- Voluntarily Opt Out of the 48 Hour Maximum Weekly Working Time Limit
Additionally, please be aware that you are required to complete a Self-Declaration Form.
The Self-Declaration form must not be sent electronically as we can only accept a signed paper copy, you will need print, complete, sign and bring this document with you to your self-booked ID appointment
Your appointment is subject to clearance by the Trust’s Occupational Health Department. You must complete a pre-placement health questionnaire; which will be sent to you electronically by email from the Trust’s Occupational Health Care Provider – Team Prevent (sent from: firstname.lastname@example.org). The questionnaire must be completed online (using the same email address to register as the received address) as soon as you receive the link and copies of your identified and validated (recognised UK/European provider clearance) TB, Measles, Mumps, Rubella, Varicella Hepatitis B (SAG), Hepatitis C and HIV pathology reports will need to be attached via the online portal.
If full Occupational Health clearance has not been granted in time for your first working day, you may not be able to commence in post, resulting in a delay to your start date and remuneration.
In your own interests and those of the Trust, you may be required during the course of your appointment to undergo medical examination by the Trust’s Occupational Health Provider or an independent medical practitioner.
